The Beneficiary Questionnaire and Income Calculation Worksheet are to be printed and faxed or mailed to WPHD at the time the grant reservation is made.

Loan Forms
The following forms are to be completed and sent to WPHD after closing with all the forms shown on the Transmittal Checklist.

Member Enrollment Forms
The Lender Subsidy Agreement and Contact Person forms (along with the $100 participation fee) must be sent to WPHD before Members can begin reserving funds. (One subsidy agreement and participation fee enrolls the Member in both the DPP and DPP Advantage programs.)
DPP Repayment Forms
The following document is to be used if repayment of the DPP Advantage subsidy is required.
