Downpayment Plus® Forms

Calculating Income

The Beneficiary Questionnaire and Income Calculation Worksheet are to be printed and faxed or mailed to WPHD at the time the grant reservation is made.

Loan Documents

The following forms are to be completed and sent to WPHD after closing with all the forms shown on the Transmittal Checklist.

The following forms are to be completed online. Completed forms can then be downloaded, printed, signed and faxed or mailed to WPHD.

Informational Documents

The following documents contain important information about the Downpayment Plus program and should be reviewed by all lenders entering DPP applications.

Member Enrollment Forms

The Lender Subsidy Agreement and Contact Person forms (along with the $100 participation fee) must be sent to WPHD before Members can begin reserving funds. (One subsidy agreement and participation fee enrolls the Member in both the DPP and DPP Advantage programs.) The Amendment Agreement must be sent to WPHD with the closing documents when the first mortgage is an FHA loan.

DPP Repayment Forms
