Loan Documents
The following forms are to be completed and sent to WPHD after closing with all the forms shown on the Transmittal Checklist.

Member Enrollment Forms
The Lender Subsidy Agreement and Contact Person forms (along with the $100 participation fee) must be sent to WPHD before Members can begin reserving funds. (One subsidy agreement and participation fee enrolls the Member in both the DPP and DPP Advantage programs.) The Amendment Agreement must be sent to WPHD with the closing documents when the first mortgage is an FHA loan.
